Most refused bookings are not about clinical fit — they are about paperwork the employer needed and could not find on the day. Have these ready before you apply.
Essential before any shift
- GPhC registration certificate. A current PDF or a register lookup screenshot. Employers verify the number against the GPhC register before payment.
- Professional indemnity insurance. A certificate showing your name, policy number, current dates, and cover amount.
- Right to work in the UK. Passport, settled status share code (older codes will be rejected), or a Home Office check link.
Often required, especially for services
- Enhanced DBS check (within the last 3 years for most employers). Multiples chains and CCG-commissioned services usually require this.
- PGD sign-offs if you will deliver a service: Pharmacy First, contraception, NMS, hypertension case-finding, travel vaccinations. Bring the PGD pack and a competency statement.
- CPP / NCSO declaration if you are a Chief Pharmacist or supervisor.
- Counter signature evidence if you have ever been refused at a previous branch.
Smart to have ready anyway
- Two recent professional references with contact details.
- Bank details and UTR for self-employed locums.
- A short bio: years registered, services confident with, branches you work regularly.
How WE4Pharmacy keeps this organised
Upload each document once. The platform tracks expiry dates, alerts you when something is approaching its limit, and shares the verified items with employers when you apply. You never re-attach the same file twice — and employers see a credentials badge on your profile before they even open your application.
